Medical Release
 
Treatment of Minors and Release of Liability
 
The undersigned, the parent(s) or legal guardian(s) of the above named minor; hereby authorize my child’s coach or any other official of Arcadia Youth Association to consent to any medical examination or treatment, including hospitalization and/or surgery, which is deemed advisable, appropriate or necessary by duly licensed physicians, emergency medical technicians, paramedics or other medical practitioners in order to properly care for my child in the event he/she sustains injury or is suffering from an illness during the course of any playing or non-playing activities of Arcadia Youth Association; provided however, the foregoing consent or authorization shall be valid only in the situation where a parent or legal guardian is not reasonably available to provide the necessary consent to medical treatment.
 
Parents wishing to purchase additional coverage will need to contact their insurance company. Athletes will be required to b e covered by a group insurance policy and should plan accordingly.
I also give permission for my child to represent Arcadia Youth Association, a competitive softball/baseball team, and to accompany his/her team to any of its local or out of town tournaments, games, practices or activities. I hereby consent to my child’s participation in any and all activities of Arcadia Youth Association, and I agree to release, indemnify and hold harmless Arcadia Youth Association, and its officers, directors and agents, from and against any liability of any kind arising out of the actives of Arcadia Youth Association or transportation to or from such activities.
 
Volunteer Deposit Program
 
8U - 14U Parents (Tball & Little Slugger players are excluded):
 
Each family is asked to volunteer time helping with concessions, cleaning, field prep, garbage clean up, coaching, umpiring, board service, etc.  
 
All families are required to provide a separate $200 volunteer check/deposit at the start of the season, dated for June 1, 2025, made out to Arcadia Youth Association.  These checks can be provided to the coach for your oldest child.  1 check per family. 
 
You are asked to provide 3 hours of volunteer time for each registered child, with a maximum of 6 hours per family.  If the family completed the required volunteer hours, the check will be shredded/discarded.  If the family does not complete the volunteer hours, the check will be deposited into the AYA general fund. 
 
If you plan to NOT complete your volunteer hours, please indicate this at the time you provide the check.
 
If you sign up for a shift but do not show up, find a replacement yourself, or provide adequate notice, the $200 deposit check will be deposted in the AYA general fund.  
 
Head Coach and Assistant coach families do not need to complete additional volunteer hours.  Volunteer coaching may qualify for volunteer hours, but is at the discretion of the head coach and the AYA board.
